Ovary transmigration of a levonorgestrel-releasing intrauterine device and ectopic pregnancy: a case report

Introduction: Intrauterine devices (IUD) are safe long-term contraceptive methods frequently used. Though it is a rare event, the uterine perforation and migration of an IUD possible. Case description: This study based on single patient who has been using Levonorgestrel (LNG)-IUD for last two years without previous complications. A control ultrasound in October/2019 reported normally placed LNG-IUD. However, July/2020, she was admitted to our hospital with intermittent pain right iliac fossa associated minor vaginal bleeding 30 days. Her hCG level 827,3 mUI/mL. The LNG-IUD not identified exam, there mass 5.6 centimeters hyperechogenic line adnexa. In surgery, we found tubal ectopic pregnancy, located within ovary parenchyma, confirmed by pathological examination. We performed salpingo-oophorectomy, were no procedure presented good clinical evolution, receiving discharge next day. Discussion: spontaneous can affect any adjacent structures, but exact pathophysiology unknown. Although complications, medical team should discuss their possibility before insertion. Moreover, professionals be aware this diagnose rapidly avoid further With case report intend review similar cases described previously best options available management complex situation.
